Medical Records

Cardiovascular Medicine is committed to protecting the privacy of your health information. We maintain your medical records to serve you and to meet legal, accrediting, and regulatory requirements. The information in your medical record is confidential.

HOW DO I OBTAIN COPIES OF MY MEDICAL RECORD?

 
Release of information from your medical record must meet legal requirements and Cardiovascular Medicine policy. All requests for release of medical records must be accompanied by an Authorization for Release of Protected Health Information form and be signed by the patient or their legally appointed representative.

  1. Copies of medical records from other facilities should be requested from the originating facility.

     

  2. Release of medical records for minor children requires the signed authorization of the parent or legal guardian.

     

  3. Medical records will be sent through the U.S. mail, unless otherwise specified. If records are requested for treatment by another physician or medical facility, they will be faxed directly to that facility. Requests received for providing your immediate or emergent medical care will be given priority.

If you desire to have the records picked up at one of our locations, a photo ID is required to verify the identity of the individual obtaining them. Records will not be released to anyone whom we are unable to verify their identity or legal right to accessing protected health information.

IS THERE A CHARGE FOR COPIES OF MY RECORDS? 
Consistent with HIPAA regulations and applicable state laws, a reasonable fee to cover the cost of copying and postage may be assessed. Please allow 7-10 business days for receipt of records sent by mail. Please contact our H.I.M. Department at either location listed above for questions and current fees for processing of medical record requests
