Welcome to Cardiovascular Medicine, PLLC. We are pleased you and your doctor have chosen CVM to assist in your cardiovascular evaluation and treatment. 

 

For more than 44 years, when patients have problems with Congestive Heart Failure, Angina, or Heart Attack, they trust Cardiovascular Medicine to help them heal, answer their questions, and get back to a normal way of life.

Our staff have confirmed with you an appointment date and time for your consultation visit. If you are unable to keep this appointment, please notify us as soon as possible.

​

In order to provide you with optimum care, please print the PATIENT REGISTRATION form and a PATIENT MEDICAL HISTORY form to be completed as fully as possible. Please bring both forms to your appointment.

​

In addition, please bring the bottles of all medications you are currently taking.

​

Plan to arrive a few minutes before your appointment time. The receptionist will ask to make copies of your insurance cards, your photo ID, and ask you to sign a treatment/release form. Please allow 1-2 hours for your consultation visit.

Initial Appointment

Thank you for choosing Cardiovascular Medicine, PLLC. to assist in your cardiovascular evaluation and treatment. To make certain that you receive the time and attention required to provide optimal care, we ask that new patients arrive 15 minutes prior to their first appointment to complete the necessary registration process. Please allow 1-2 hours for your consultation visit.

To help us provide you with optimal care, if you have been hospitalized or had cardiology testing at another facility, please have copies of the reports sent to our office prior to your initial visit. 

Financial Information

BILLING 
Cardiovascular Medicine, PLLC will gladly submit all claims to all carriers. We participate with many insurance plans, including Medicare, will be happy to answer any questions regarding your insurance plan. Plans change frequently; please contact our Patient Accounts Department to verify participation in your plan.

Please bring current insurance cards with you to your appointment. All co-payments and patient balances are due at the time of service. For your convenience, we accept personal checks, Discover, MasterCard, and Visa.

Insurance companies do not always pay for all the medical services you receive. You will be responsible for payment of any services not covered by your insurance policy.

PRE-CERTIFICATION 
Most insurance carriers require advanced notification for hospital admissions and outpatient testing. Cardiovascular Medicine, PLLC will assist with obtaining pre-certification; however, it is the responsibility of the patient to know his/her insurance regulations and to notify his/her insurance carrier per their guidelines. Patients may be asked to sign a waiver accepting responsibility for non-covered services. Failure to notify your insurance carrier may result in a reduction or denial of benefits.
